Pause the first conclusion long enough to hear and examine what happened. This does not remove accountability or prevent a later judgement. It prevents an immediate reaction from closing down information before the situation has been understood.
Explore the information, conditions, pressures, assumptions, systems and earlier decisions that shaped what seemed possible or reasonable at the time. Understanding context explains an action; it does not automatically excuse it.
Create the honesty, curiosity and fair accountability required to learn what actually happened and what should improve. Learning becomes possible when people can contribute relevant information without the process becoming either a search for blame or a way to avoid responsibility.
Strengthen work, systems and conditions so foreseeable errors are less likely, easier to detect and correct, and less able to develop into serious consequences. This can include equipment, interfaces, procedures, checks, safeguards, staffing, competence, decision rights and recovery options. It avoids relying on perfect human performance as the only protection.
Make discussion of mistakes, uncertainty, weak signals and unwelcome information part of normal work. People should be able to admit an error, ask for help, express doubt and offer a different perspective before the evidence becomes undeniable.
